Register for Programs

Participants looking to sign up for a Park and Recreation program must fill out a program registration form and have full payment before the posted registration deadline.

Recreation payment can be made one of four ways:

1. Registeronline by clicking HERE and pay by credit or debit card.
2. Mail-in form with check or cash to: Wisconsin Dells Parks and Recreation, 300 La Crosse Street, Wisconsin Dells, WI 53965
3. Office walk-in at the Rec Center, 722 Michigan Avenue, Wisconsin Dells, WI 53965.  Fill out form and pay with check or cash.
4. Sign -up at the Activity Registration Night at the appropriately listed time and location. Fill out form and pay with check or cash.
   
   We now have one registration form for all of our programs. CLICK HERE to access the registration form. Simply fill one out for each of your children and put all of their programs on there. Please fill out the concussion form on the back as this is mandatory by the state now. You will need to have one on file annually. If you have any questions on this registration form please call the Parks & Rec office.

      The only program that you must use a separate registration form for is Kidz Klub - you can find those registrations forms on the right hand side of this page or on our Kidz Klub page.
